EMERGENCY PHONE CARD PREPARATION
Please bring an emergency phone card with you for each day you go on an open-water dive. You cannot get credit for the dive until the card is correct!
The card must be on a 3 x 5 inch Index Card. Because this standardizes the procedure, there is no doubt as to what to look for in the event of a real emergency. It may be done using a computer, handwritten, or using a prinout from this sheet and than reducing it on a copier.
INSTRUCTIONS FOR FILLING OUT THE CARD:
If you are diving in LAKE MOHONK: On the front side of the card, clearly written -
DAN: (919) 684-9111
Rescue Squad: 255-1000 (Call the Mountain House, ask for Security)
Police: 255-1000 (Call the Mountain House, ask for Security)
On the back side of the card, clearly written -
Directions to dive site: Tell the Mohonk Department of Security where on the lake the emergency is. (Most likely it will be centered at Open Camp.) Security will respond with First Responders. They may need the oxygen from Deep-Six. Mohonk Security will also summon additional aid if needed, such as the New Paltz Rescue Squad and/or the police.
Directions to the hospital: Leave Mohonk Gatehouse and take a right on to Mountain Rest Road. Continue to the second stop sign which will be at the end of the road and take a right on to Springtown Road. Go to the end and take a left on to Rt. 299. Proceed through New Paltz to the end of 299 in Highland. Take a right and follow Rt. 9W to the Mid-Hudson Bridge. Right on to the bridge approach and then right after the bridge on to Route 9. Follow the blue hospital signs to Vassar Bros. Hospital.
If you are diving at HEARTHSTONE CAMPGROUNDS IN LAKE GEORGE: On the front side of the card, clearly written -

Directions to Glens Falls Hospital (100 Park
St; Glens Falls, NY 12801):
Go left (south) at Hearthstone Campground exit on to Route 9N (Lake Shore Dr).
In 1.5 miles go right on to the I-87 S (Exit 22). Exit I-87 at Exit 20. Turn
left (east) on to Route 23. Turn right on to US 9. In 5 miles go right on to
Hudson Avenue. In 0.2 miles take a left on to School St. Turn right on Park
St. and go to the hospital (100 Park).
Directions from the Glens Falls Hospital to
Hearthstone Point:
Go to I-87 North at Exit 20. Leave I-87 at Exit 22. Go to Route 9N north. In
2 miles the Campground entrance is on the right.
If you are diving in DUTCH SPRINGS: On the front side of the card, clearly written -

Directions to the dive site: From Lehigh Valley Hospital-Muhlenburg: Go north on US 22 to SR 191 (Nazareth Pike). Left on SR 191. Go to Hanoverville Rd and take a left. Dutch Springs is 4733 Hanoverville Rd on the left. Total distance is 7 miles from the hospital to Dutch Springs.
Directions to the nearest hospital: Leave Dutch Springs entrance and go right on Hanoverville Rd. Go to SR 191 (Nazareth Pike) and take a right. Go south to US 22. Go SW on US 22 to Schoener Rd. Go S on Schoener Rd and Lehigh Valley Hospital is close to 22. Total distance is 7 miles from Dutch Springs to the hospital.
If you are diving in WILLIAMS LAKE (Rosendale): On the front side of the card, clearly written -

On the back side of the card, clearly written -
Directions to the dive site: Proceed west from Main Street in Rosendale, go under the train trestle to the first right. Take that right on to Binnewater Road. Proceed to the entrance to William's Lake Resort (comes off straight from Binnewater). Follow the William's Lake Resort road to the resort.
Directions to the nearest hospital: Leave on the William's Lake Resort Road on to Binnewater Road - go straight. Take a left at the end of Binnewater Road. Proceed through Rosendale (Main Street) and take a left at the end on to Route 32. Follow Route 32 into Kingston. When at the 6 corner intersection, go toward the traffic light(s). At the second light (blinker) take a left. At the next light, take a right staying on 32. Follow that to Broadway. Take a right on Broadway and follow that until the Kingston Hospital is visible on the left.
If you are diving in LAKE MINNEWASKA: On the front side of the card, clearly written -

Directions to the dive site: From the Kerhonkson intersection of Route 209 and Route 44/55, proceed east up the Shawangunk Mountain. At the Lake Minnewaska Park entrance take a right. Proceed to the section of the lake where the emergency is.
Directions to the nearest hospital: Leave the Minnewaska State Park and take a left on 44/55. Follow 44/55 west to the bottom of the mountain where Route 209 and 44/55 meet. Take a right on to Route 209. Get off Route 209 at the Kingston Exit. Round the circle to Chandler Drive. Take Chandler Drive to the traffic light. Go straight through the light and the road will curve to the left on to Broadway. Follow Broadway to the east until Kingston Hospital is on the left.

Directions to the nearest hospital: Vassar in Poughkeepsie: Take Sylvan Lake Rd (at dive site) north to SR 82. Go on the Taconic State Pkwy N to Noxon Rd exit. Right on Noxon Rd (N) 5 miles to Rt. 55 (left turn) to Rt 44 Left on to S. Hamilton. Right on Montgomery St, left on Academy St, right on Franklin St, left on South Ave, right on Dean Place, left on Lincoln Ave and proceed to the hospital.
Directions from Vassar (nearest hospital) to the dive site: Left on to Lincoln
Ave, right on Dean St, left on South Ave, right on Franklin St, left on Academy
St, right on Montgomery St, left on South Hamilton St, right on Route 44, stay
right on Route 55. In 1.5 miles go right on Noxon Rd. In 5 miles go south on
the Taconic State Pkwy. Second exit at SR 82 North. Go right (north) for about
300 yards to Sylvan Lake Rd. Take a right and go until you see the dive site
on the right. It is on the SE side of the lake and is identified by a DEC sign.
